== Justification == Topical drug delivery is a route of administering drugs via the skin with the intent to provide therapeutic effects. This system usually provides a local effect on certain organs. In ancient times, people used herbs on wounds for relieving the inflammatory effect or as pain relievers. There are numerous dosage forms that can be used topically, including cream, ointment, lotion, patches, and other forms. The topical use has many advantages: avoiding first pass metabolism which can increase its bioavailability, being convenient and easy to apply to a large area, being easy to terminate the medication, and avoiding gastrointestinal irritations. All these can increase patient compliance. However, there are several disadvantages to this system – causing skin irritations and symptoms like rashes and itchiness may occur. Also, only small particles can pass through the skin, which limits the choice of drugs. Since skin is the main medium of the topical drug delivery system, its conditions determine the rate of skin penetration leading to affecting the pharmacokinetics of the drug. The temperature, pH value, and dryness of the skin need to be considered. There are some novel topical drugs in the market which can utilise the system as much as possible. This localized system provides topical therapeutic effects via skin, eyes, nose, and vagina to treat diseases. The most common usage is for local skin infections.

===== Morphology change ===== As shown by flow cytometry and electron microscopy, the most sensitive sign of activation, when exposed to platelets using ADP, are morphological changes. Mitochondrial hyperpolarization is a key event in initiating morphology changes. Intraplatelet calcium concentration increases, stimulating the interplay between the microtubule/actin filament complex. The continuous changes in shape from the unactivated to the fully activated platelet are best seen via scanning electron microscopy. The three steps along this path are named early dendritic, early spread, and spread. The surface of the unactivated platelet looks similar to the surface of the brain–a wrinkled appearance from numerous shallow folds that increase the surface area; early dendritic, an octopus with multiple arms and legs; early spread, an uncooked frying egg in a pan, the "yolk" is the central body; and the spread, a cooked fried egg with a denser central body. These changes are all brought about by the interaction of the microtubule/actin complex with the platelet cell membrane and open canalicular system (OCS), which is an extension and invagination of that membrane. == Personal life == Gallrein lives in Shelbyville, Kentucky. He has been married twice. In December 1985, he married Mary E. Stuart. According to court filings, Gallrein filed to divorce his second wife, Heather Gallrein, in 2024 (one month after losing the Kentucky state legislature election).
